Background
We are seeking a highly motivated and compassionate Primary Care Provider to join our dynamic team. The selected candidate will play a crucial role in delivering routine primary care and chronic care management services. As a Primary Care provider, you will work closely with our interdisciplinary team of healthcare professionals to provide comprehensive and personalized care to our members. The ideal candidate has great people skills, strong communication skills. and a strong background closing care gaps for patients with chronic conditions.

Key Responsibilities:
- Provide routine evidence-based clinical care, including medication management, diagnostic testing, and preventive interventions.
- Conduct thorough assessments of patients' cardiovascular health and behavioral risk factors.
- Review remote monitoring data including blood pressure, weight, pulse oximetry, blood sugar and exercise
- Develop personalized care plans in collaboration with cardiologists and other team members.
- Counsel patients on chronic care management, lifestyle modifications, and behavioral risk reduction strategies.
- Monitor and adjust care plans based on patients' progress and feedback.
- Communicate and Interface with referring physicians about individual patient outcomes and progress toward goals
- Review reports on patient outcomes and contribute to quality improvement initiatives.
- Maintain accurate and up-to-date electronic health records for each patient.
Qualifications
- A highly compassionate and empathetic person
- Primary Care Provider, Internal Medicine, Family Practice with at least 5 years of practice experience and expertise in, preventive medicine, exercise training, and chronic care management (including smoking cessation, nutrition, stress reduction, medications and lipid management)
- Extremely comfortable in the use of information technology systems including electronic medical records, telehealth platforms and ability to collaborate with team members and stakeholders through digital platforms (Skype, Zoom, Email, etc.)
- Strong leadership and communication skills, including both written and verbal
- Medical degree, either an M.D. or D.O.
- Licensure in good standing to practice medicine in state of residence
- Training in basic life support or advanced cardiac life support
- Possess all necessary insurance including medical malpractice and liability
- Credentialing - You will be credentialed to be a provider under the Movn Health Medical group
- Privileges - Possess hospital / admitting privileges at a local hospital in your region
