Background

Movn Health is hiring a contracted Advance Practice Provider to oversee patients in our Movn Virtual Cardiac Rehab Program (5-10 hours/month). The successful candidate will provide Evaluation and Management telehealth visits as needed to gain referral for RPM and CCM programs to assist with the company’s mission of increasing access to high quality cardiovascular care. The ideal candidate has great people skills, strong communication skills and a strong background in clinical cardiology or primary care medicine.

Responsibilities:

Primary:

• Provide Evaluation and Management telehealth visits to gain referral for RPM and CCM programs

• Serve as physician champion/liaison for our remote cardiovascular risk prevention program for the region

• Work with our clinical team to ensure that all patients are being appropriately treated and triaged as needed

• Communicate and Interface with referring physicians about individual patient outcomes and progress toward goals

Secondary:

• Engage leadership at health systems, health plans, department of health, physician practices, Cath labs on the need for alternate delivery models for cardiac rehab and educate them on the benefits of the Movn program


Qualifications:

• Nurse Practitioner or Physician Assistant

•Board certified with Master’s degree or Graduate of an accredited Physician Assistant Program

•Current Nurse Practitioner or Physician Assistant License in Massachusetts and New York

•Experience in cardiac care

•Experience in remote patient monitoring and chronic care management preferred

•A highly compassionate and empathetic person
